What Is Manipulative Therapy? | Hands-On Physiotherapy Explained

Understanding How Manipulative Techniques Help Relieve Pain and Restore Movement

One of the tools physiotherapists commonly use to treat a variety of problems is manipulative therapy. However, many patients have a poor understanding of what it is and why it is used.

Your questions answered

What is Manipulative therapy?

Manipulative therapy, sometimes referred to as “Hands-on therapy,” involves the skilled use of the hands to restore movement, relieve pain, and improve function in joints and soft tissues.

According to the Oxford Concise Medical Dictionary:
“The use of the hands to produce a desired movement or therapeutic effect in part of the body… to restore normal working to stiff joints.”

This approach is not limited to joints alone—it also extends to mobilising soft tissues (like muscles and fascia) and neural structures (such as nerves), helping to improve overall mobility and reduce pain.

What about Spinal manipulation?

There’s a common myth that Spinal manipulation “puts bones back into place.” In reality, Spinal manipulation aims to improve movement and reduce pain—it does not physically relocate bones. The “click” or “pop” sound often associated with these techniques is simply gas being released from the joint, similar to cracking your knuckles.

Are There Different Types of Manipulative Therapy?

Yes. Manipulative techniques range in intensity and purpose:

  • Passive mobilisations – Gentle, rhythmic movements applied to a joint to gradually improve mobility.
  • High-velocity thrust techniques – Quick, controlled movements often resulting in an audible “click” in the joint.
  • Soft tissue techniques – Including massage, myofascial release, and trigger point therapy, aimed at reducing muscle tension and improving tissue health.

Each technique is carefully selected based on your condition, symptoms, and overall health.

Is Manipulation Effective on Its Own?

Hands-on therapy can be very effective—especially in the early stages of treatment. However, research and clinical experience show that combining manipulative therapy with other approaches such as:

  • Exercise therapy
  • Education and self-management strategies
  • Other modalities like dry needling or therapeutic ultrasound

…delivers the best long-term results. Our goal is not just symptom relief, but lasting improvement in function and prevention of recurrence.

What Conditions Can It Help?

Manipulative therapy can be helpful in treating a range of conditions, including:

  • Neck and back pain
  • Stiff joints
  • Sports injuries
  • Postural dysfunction
  • Shoulder, hip, or ankle restrictions

If movement is restricted and that restriction is contributing to pain or dysfunction, manipulative therapy may play a key role in your recovery.

Is Manipulative Therapy Safe?

When performed by a qualified physiotherapist, manipulative therapy is generally safe. However, like any treatment, it’s not suitable for everyone. There are known contraindications and risks, particularly in individuals with:

  • Osteoporosis or weakened bone structure
  • Certain vascular conditions (e.g., vertebral artery issues in the neck)
  • Acute inflammation or infections

Rare complications—such as stroke associated with neck manipulation—have been reported, but are extremely uncommon. At Physio One, our clinicians carefully screen every patient to ensure all treatment approaches are appropriate and safe.

Over-reliance on passive treatments (like manipulation alone), without addressing underlying causes, can lead to poor long-term outcomes. That’s why we always combine hands-on treatment with active rehabilitation strategies.
